Why would my son's tubes fall out of his ear after 2 months?

I found out today that one of the tubes my son had placed in his ear 2 months ago has already fallen out. My pediatrician says he has never seen one fall out this early (they usually stay in place 6 months-2 years). I’m going back to the ENT Friday and need to know if it could be something he did incorrectly during the surgery. THanks
