Why is walking pneumonia not improving in 2 weeks already?

My friend had a cough, went to the Dr got diagnosed with Walking Pneumonia, is been on antibiotics and an inhaler for almost 10 days and no signs of improvement. The cough is gone but the affected lung it’s still congested and makes a noise when breathing. He is going for x-rays tomorrow. Any ideas what would happen next?
Really quick, he smokes cigars on a regular basis (for years) but hasn’t since the Dr told him not to.
