Where do I find sample professional development plans?

What kind of an organization is this for?

I've got a lot of experience in Professional Development / Leadership Development plans in not-for-profit healthcare (I helped create the Leadership Development Plan for Group Health in Seattle) and I wish I could say it's as simple as a template. Unfortunately, it's not : (
