Have you been treated for toenail fungus?

If you or someone you know has been treated for toenail fungus, why did you seek treatment (for appearance sake or something else)? What kind of treatment did you get, what was the outcome, was it covered by insurance and what was the cost?

I’ve read that people with toenail fungus are sometimes denied major surgeries like organ transplants until the fungus is cured. Is that correct?
