Should I use “Edited to change answer” or another phrase?

You can just put “Edited” in front of it.
It’s not a good idea to completely change a question in editing. The process of writing a question gives multiple opportunities to edit the work before posting the question to the site. The vast majority of all editing should take place during this time. Completely changing a live question results in a great deal of confusion.
^^ I think OP is asking about editing the answer.
^^Well, I am bumfuzzled. :)
If no one has answered or looked at it yet, and you are able to make a minor change quickly (spelling mistake, etc), I wouldn’t say anything. When I had to use the iPhone link to ask a question, I sometimes had to quickly make a change once I posted it, because the format wouldn’t allow me to preview my question. If I’m changing a fact, sentence or something that would make a person change their answer, I would add “edit” in bold letters either at the end of the question or to whatever part of the question the edit applies.

Okay. By the way, I have put it in my detail part, I would just put Edited to change answer if I needed to. I haven’t yet.
@flo Yeah, just “edited” is fine. That let’s us know that you changed something after the fact : )
It’s not just something, it’s everything.
@flo, even if you change everything, you can still use “edited” and that’s still good.
Good is a relative term though.
I’ve done it writing a Question, when done I should have put,
“I screwed up big time on the question!”
