What would be good or bad about having a twin off the grid?

Suppose someone gave birth to twins, and no doctor was aware of the duplicity? Let’s say they are born at home unintentionally, and nothing had come up during doctor visits which indicated twins.

What might be the advantages and disadvantages of keeping one off the grid by having them share the same identity?

For instance, keeping medical records and treatments would likely be problematic.
