If it’s asking for a PUK then you need to contact your service provider for the code. PUK stands for PIN unblocking key. Its used to uknlock the phone if you’ve entered the PIN incorrectly to many times.

you have to call your provider and get the code, don’t try to guess it, because some cell phones have a feature where if you enter a wrong code too many times, it will dissable it’s self and you might not be able to unlock it ever again.

You don’t unlock a sim. Sim cards are not locked.

Phones on different networks are locked solely to that network until the phone itself is unlocked. The method to take is different depending on the phone model and network.

Cheeb is right. In most cases you only have 4–6 tries to enter the correct code. If you try to enter the code the max times unsuccessfully, the phone will lock forever.

My advice is to take the phone to someone who specializes in unlocking phones.

Sim cards can be locked.

I guess it depends on the type of lock you mean. Original poster, please clarify.

A sim card can only be ‘locked’ on a user level. This means that you (or someone) placed your sim card in password protect mode. I guess it’s possible that someone got a hold of courtney’s phone and added a password to it. That is technically ‘sim card lock’. This will also fry your sim card if you put too many wrong passwords in.

I had assumed that no one would lock courtney out of her own phone and if they had, courtney would have called her phone company instead of posting here. Maybe that was faulty reasoning.
