Why does my dog run?
I have a 3 year old toy poodle…he is very intelligent and learns new tricks in minutes. He is very obediant until I open the front door. Sometimes when I come home with groceries he sneaks out the door and runs! I’m scared he will get hit by a car even though he returns home.
I even hired a trainer to come to my house every two months to train him with his issue (I also train him alone in between time), but no progress. It’s been almost a year that the trainer and I tried training him not to run the front door.
Any ideas why?

Because he’s not getting enough exercise. He’s shut inside all day, what do you expect?

Do you take him for long walks?
Just wondering if you had the dog since he was a puppy or not. Adopted dogs tend to run out a lot in the beginning. Not sure if they are looking for previous home or what. Generally dogs that you have from pups tend to do this less.
It can be so many reasons. How old was the dog when you got it? My akita was 3 and prior to my owning him he was tied to a tree or penned in a small back yard, so when he had the opportunity to be free,he took it! I could never allow him off lead. I do now because he is 12, he will still take off but I can keep up with him and he gets tired before I do.
Some dogs are just never trustworthy off lead. But the running out the door is a problem that should be fixable. Work on that first. I never let the dogs go out the door before me, they must wait until I am out and call them. That is a good thing to teach them. I would also get into an obedience class and really work on the stay and recall
And of course as was said above, the walks and exercise is always important.
I got my dog when he was two and i walk him once a day around my community or park depends on my schedule.
