What do you think should people really have to wish them on their birthday?
It’s consuetude or custom that everybody give good wishes to birthday boy or girl on their birthday.
but technically speaking they grow up by 1 year & the number of years get reduced from their entire life.

Such is life. Horrible and macabre, we just live to eventually die.
In said case, give them money.
I wish them to grow in wisdom. The reason we wish them well is to wish them to stave off death for another year. Like @Symbeline said, macabre but true.
Well, I tend to say to people: Congratulations, the Earth has completed another revolution of the sun and you’re not dead yet.
My grandmother Suzanne just celebrated her 102nd birthday.
She is in remarkable health for a woman of 75!
She has been telling me for 30 years that she has lived too long!
She has friends and they appreciate her and she had a nice party to celebrate her special day.
I love her even though my mother, her daughter, has not spoken to her for at least 30 years.
I danced with her on her 100th birthday!
I always liked what they say in Winnie the Pooh : “Many Happy Returns”
Whoa! Such dark talk. What you are celebrating when you give birthday wishes is the anniversary of the person’s arrival on the planet.

However, I would wish a friend continuing friendship.
What do you say?
We should say to the celebrated person ‘Hey dude,I’m sorry to tell you but you have one less year to live so I wish you to live as long as possible’
It is a celebration of making it through another year.
Congratulations! You made it through another year without dying or blowing up the planet!

My grandparents always said “I’m glad you were born” when they called on our birthdays.
