Does the iPad have a text-to-speech?

Or you need to have a special app for that? If it does is it in English language only?!

Not natively (at least not that has been announced.) But there are iPhone apps that do text to speech, so I would say it can be done. Not sure about other languages than English though.
Yes it does. It’s in accessibility settings.
dragon naturally speaking has an app, ive used it on pc; it’s really good
There is a free app called Dragon Dictate that just cake out recently.
Obviously it’s from the same company which produces Dragon Naturally Speaking for computers ( which also costs major bucks. )
