How do I add a LIKE and FOLLOW link on my tumblr posts?

Hello,
i’m currently using the following theme:
http://safe.tumblr.com/theme/preview/437
and was wondering how i can manually add a like heart with showing how many people liked or commented on my post beside it?
kind of like the following theme:
http://safe.tumblr.com/theme/preview/515
with it showing ”# notes”.
possibly below the date it was posted.

i’m honestly not the greatest at all this stuff. so that flew right over my head lol. i DID give it a try though, and nothing changed.
