Do you suffer from "Tip of the Tongue" amnesia?

What is that? Is there a name for it? The older I get, the more I suffer from it. I’m not kidding, I could be calling you by your name all day long, but when required to introduce you to someone, your name eludes me.

How frustrating! And embarrassing. And possibly offensive!

Things I know, and never think twice about, they slip my tongue at certain critical times. Why? I know these things very very well, yet sometimes they just leave me.

Is there a name for this phenomenon?
